To apply to Holy Name Catholic School, complete the following forms:

Bring a copy of the following to the school office:
- Child’s birth certificate
- Immunization records
- Baptismal records (if applicable)
- Previous school records (if applicable)
- Check or money order for $100 non-refundable deposit

To apply for Free & Reduced Lunch
We encourage families to apply for financial assistance through CHOICE if you live in Marion County and this is the first time your child has attended a non-public school. We also provide tuition assistance to those who qualify through PSAS.

Media Consent Policy

We encourage all families to plan on attending each and every day. Abiding by the 2012-13 School Calendar* can help us achieve perfect attendance.  Students are responsible for bringing a number of  supplies for their use, and for the use of the classroom as a whole. We provide delicious and healthy lunches with many options every day.

*A more comprehensive calendar will be available in June.

Medical
For your child to be seen by our full-time school nurse, please complete the Health Consent form. Students may have non-prescription medication in the nurse’s office for her to administer throughout the day with Consent. The nurse can also give students their prescription medicine with proper Consent.
